Player Details:

DIVISION DESCRIPTIONS

Competitive 7 vs 7

Fee $140 per team
Minimum Age: Highschool to Adult aged - 
Minimum number of players per team 7; suggested teams of at least 10 players
Competitive teams play on a larger court with larger goals. Recommended division for those who are adept at ball handling skills with ability to move fast. Suggested experience, 10 years of play or more.

Intermediate Non Competitive 4 vs 4

Fee $70 per team
Minimum number of players 4; suggest 6 to 7 per team.
Minimum age - 7th Grade and Older
Skilled and non-skilled players welcome. The small court encourages short passing and quick feet.  

Youth  

Fee = $10 per individual
Minimum Age 1st Grade and Older. 
No experience necessary. 1 hour sessions led by an instructor. Sessions include drills and scrimmages with no formal teams. Tennis shoes and shin guards recommended.  

 

* Captains are responsible for relaying information to their team about game times and cancelations. They are also expected to collect the fee total for their team. Fees must be turned in by the first season game. Fees are collected by League Adminstrators at the High School.

WAIVER OF LIABILITY

In consideration of being allowed to participate in any way in the Futsal program, related events and activities, I, the undersigned, acknowledge, appreciate, and agree that:

1. The risk of injury from the aforementioned activities is significant, including the potential for permanent injury or death.

2. I knowingly and freely assume all such risks, both known and unknown, even if from the negligence of the releasees or others, and assume full responsibility for my participation.

3. I willingly agree to comply with the terms and conditions for participation. If I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.

4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless Futsal, Parks and Rec, Richland Center School District, officers, officials, agents, and/or employees, other participants, sponsors, advertisers, and, if applicable, owners and leasers of the premises, from any and all claims, demands, losses, and liability arising out of or related to any INJURY, DISABILITY, OR DEATH I may suffer, or loss or damage to person or property, whether arising from the negligence of the releasees or otherwise to the fullest extent permitted by law.

 

I understand and fully acknowledge that by accepting this Agreement, I am waiving certain rights, including the right to recover damages, due to injuries, loss, or death. 

By signing below, I hereby warrant that I am at least eighteen (18) years of age, that I have read this Agreement in full and carefully understand the terms and conditions relevant hereto. I have had the opportunity to clarify anything ambiguous and have raised my questions to a representative and by which such questions were answered to my satisfaction. 

By Signing this form you are requesting to register for the Richland Center Futsal League and agree to adhere to the league rules. 

FOR PARENTS/GUARDIANS OF PARTICIPANTS UNDER AGE 18 AT TIME OF REGISTRATION:

This is the certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releasees, and, for myself, my heirs, assigns, and next of kin. I release and agree to indemnify and hold harmless the Releasees from any and all liability incidents to my minor child’s involvement or participation in these programs/activities as provided above, even if arising from the negligence of the releasees, to the fullest extent permitted by law.
